The successful applicant will join our Senior Management Team to provide leadership and direction to our Support Teams in the North Wales area with the emphasis being on delivering flexible and responsive person centred support.

37 hours a week

£25,000 pa

Wrexham based

A Level 5 (or equivalent) qualification in Leadership and Management for Health and Social Care or working towards is essential for this position along with experience of leading and managing a team and removing barriers.

We are looking for motivated, enthusiastic and committed individuals to become part of our team in the Wrxham Area.

The role is varied and rewarding, supporting citizens in their homes and community.

You will need:-

Clear and confident communication skills<

A ‘can do’ attitude and willingness to assist with personal care<

The ability to remain calm under pressure

A genuine interest in helping people

A full UK driving licence and transport available for work

To be DBS checked and hopefully registered on the DBS update service

 

Your role: –

  • Create and maintain positive relationships with Citizens and family carers.
  • Promote the health, well-being and independence to the Citizen.
  • Develop the independence of Citizen through care plans/ short term enablement plans.
  • Provide general advice and guidance to customers to help them to make informed choices.
  • At some times assist Citizen with dignified and intimate personal care needs.
  • Engage with Citizen, family carers and others to promote alternative ways to meet and maintain their long-term needs.
  • Promote a range of services to help people live independently and safely at home.
  • Monitor Citizens progress against care plans and complete records for the ongoing review process.
